Independent Health (IH) Enrollment
Independent Health (IH) requires provider enrollment before claims can be submitted. As part of their enrollment process, IH assigns each provider a unique 3-character code that must be entered into the ClaimRev system.
Enrollment Overview
Before submitting claims to Independent Health, providers must complete the IH enrollment process. This includes filling out and submitting two required forms to IH. Once processed, IH will issue a 3-character provider code.
That code must be stored in our system under the payer settings for Independent Health to ensure claims are routed and processed correctly.
Required Steps
- Complete both IH enrollment forms (see below).
- Submit the completed forms to Independent Health using their instructions (typically via email or fax—verify current method).
- Await confirmation from IH and issuance of the 3-character provider code.
- Email ClaimRev with the code.
Required Forms

Additional Notes
- The 3-character code is provider-specific and is required for claims to process correctly.
- If the code is missing or incorrect, IH will reject the claims.
- If the provider already has a code from a previous billing setup, that code may still be valid—contact IH to verify.
