====== Independent Health (IH) Enrollment ====== Independent Health (IH) requires provider enrollment before claims can be submitted. As part of their enrollment process, IH assigns each provider a unique **3-character code** that must be entered into the ClaimRev system. ===== Enrollment Overview ===== Before submitting claims to Independent Health, providers must complete the IH enrollment process. This includes filling out and submitting **two required forms** to IH. Once processed, IH will issue a 3-character provider code.
